Contributions of traditional and HIV-related risk factors on non-AIDS-defining cancer, myocardial infarction, and end-stage liver and renal diseases in adults with HIV in the USA and Canada: a collaboration of cohort studies.
Adults with HIV have an increased burden of non-AIDS-defining cancers, myocardial infarction, end-stage liver disease, and end-stage renal disease. The objective of this study was to estimate the population attributable fractions (PAFs) of preventable or modifiable HIV-related and traditional risk factors for non-AIDS-defining cancers, myocardial infarction, end-stage liver disease, and end-stage renal disease outcomes.

Baseline, time-updated, and cumulative HIV care metrics for predicting acute myocardial infarction and all-cause mortality.
The researchers studied prediction rates of myocardial infarction in those with HIV. They found that the Veterans Aging Cohort Study (VACS) Index provided better acute myocardial infarction and mortality prediction than CD4 count and HIV-1 RNA, concluding that current health determines risk more accurately than prior history.

Validation of Medicaid claims-based diagnosis of myocardial infarction using an HIV clinical cohort.
The investigators aimed to validate claims-based myocardial infarction (MI) algorithms in a Medicaid population using an HIV clinical cohort as the gold standard. Studying 1063 individuals over 2.5 years, 17 had an MI. Specificity ranged from 0.979 to 0.993 with the highest specificity obtained using the ICD-9 code 410.xx in the primary or secondary position and a length of stay greater than 3 days. Sensitivity of MI ascertainment varied from 0.588 to 0.824 depending on algorithm. They recommended that the type of algorithm used be prioritized based on study question and maximization of specific validation parameters that will minimize bias while also considering precision.

Comparison of risk and age at diagnosis of myocardial infarction, end-stage renal disease, and non-AIDS-defining cancer in HIV-infected versus uninfected adults.
The objective of this study was to compare the median age at, and risk of, incident diagnosis of 3 age-associated diseases in HIV-infected and demographically similar uninfected adults. It found that HIV-infected adults had a higher risk of these age-associated diseases (myocardial infarction, end-stage renal disease, and non-AIDS-defining cancer), but they occurred at similar ages to those without HIV.

Lessons learned from the design and implementation of myocardial infarction adjudication tailored for HIV clinical cohorts.
In this study, a team of researchers developed, implemented, and evaluated a myocardial infarction (MI) adjudication protocol for cohort research of human immunodeficiency virus. They found that central adjudication is crucial and that clinical diagnoses alone are insufficient for ascertainment of MI. Over half the events ultimately determined to be MIs were not identified by clinical diagnoses.
