The impact of health literacy on shared decision making before elective surgery: a propensity matched case control analysis.
The primary aim of this study was to evaluate a possible association between health literacy and decision conflict in surgical patients. Patients undergoing a diverse number of elective surgeries were enrolled in the study. Health literacy was measured using the Newest Vital Sign instrument and decision conflict using the low literacy version of the Decision Conflict Scale. The investigators found that poor health literacy is associated with greater decision conflict in patients undergoing elective surgical procedures.

A closer look at health information seeking by older adults and involved family and friends: design considerations for health information technologies.
Older adults are the largest consumers of healthcare. As part of a broader study of personal health information management (PHIM), the investigators interviewed older adults in King County, Washington, and their involved family and friends (FF), regarding health information (HI) sources they seek and utilize. The authors indicated that design considerations include: facilitating access to quality provider-vetted HI, incorporating older adults and FF in the design process, and creating shared spaces for communication of HI among older adults, FF, and providers.

Informing, reassuring, or alarming? Balancing patient needs in the development of a postsurgical symptom reporting system in cancer.
After ambulatory surgeries, patients who recover at home have multiple questions about wound healing, symptoms and medication side effects, and recovery expectations. In this study, the investigators conducted user testing and rapid application development of a symptom reporting system that supports home-based recovery by inviting patients to self-report symptoms in the days after surgery and receive an immediate feedback report giving context for their reported symptoms.

Patient perceptions of barriers to self-management of breast cancer-related lymphedema.
This article reports on results from a study which used an IRB-approved focus group and mailed surveys to identify barriers to lymphedema self-management, definitions of education and support from breast cancer survivors with lymphedema, types of education and support they had received, what kind of education and support they wanted. Lack of education about lymphedema treatment and risk reduction was identified as a main barrier. Women’s responses also make it unclear whether or not they were exposed to support options other than medical treatment.

Improving the understanding of test results by substituting (not adding) goal ranges: web-based between-subjects experiment.
Most displays of laboratory test results include a standard reference range. For some patients (eg, those with chronic conditions), however, getting a result within the standard range may be unachievable, inappropriate, or even harmful. The purpose of this study was to test the impact of including clinically appropriate goal ranges outside the standard range in the visual displays of laboratory test results.

Beyond Dr. Google: the evidence on consumer-facing digital tools for diagnosis.
Direct-to-consumer (DTC), interactive, diagnostic apps with information personalization capabilities beyond those of static search engines are rapidly proliferating. Little is known about the state of the evidence on the performance of these apps or the methods used to evaluate them. In this paper, the authors conducted a scoping review of the peer-reviewed and gray literature for the period January 1, 2014-June 30, 2017 to examine the current evidence base on DTC, interactive diagnostic apps.

Let's talk numbers: a qualitative study of community-dwelling US adults to understand the role of numeracy in the management of heart failure.
This qualitative study examined the role of numeracy (basic number skills) in the management of patients with heart failure (HF). Thirty men and women aged 47-89 years with a history of HF were recruited from an urban academic primary care practice. Participants all had a history of HF within the past year, were seen at the practice within the last year, and had been hospitalized for HF within the last 6 months. They were interviewed about their numeracy to help manage monitoring weight, maintaining a low-salt diet, and monitoring blood pressure. A wide range of knowledge and understanding was found and fear served as a barrier and facilitator to carrying out HF self-care tasks involving numbers. If the patient has a caregiver who also lacks those skills or does not have HF care training, patients may not be managing their HF as well as they should.

Inflammatory bowel disease telemedicine clinical trial: impact of educational text messages on disease-specific knowledge over 1 year.
The authors aimed to determine if delivery of educational messages through a telemedicine system improves inflammatory bowel disease (IBD) knowledge. They found that, although telemedicine improves IBD-specific knowledge through text messaging, telemedicine is not superior to education given through standard visits at referral centers.

Patient perspectives on racial and ethnic implicit bias in clinical encounters: implications for curriculum development.
Patients describe feelings of bias and prejudice in clinical encounters; however, their perspectives on restoring the encounter, once bias is perceived, are not known. In order to inform the design of novel patient-centered curricular interventions, this study explored patients' perceptions of bias, and suggestions for restoring relationships if bias was perceived. The investigators concluded that participant lived experience and physician behaviors influence perceptions of bias, however clinical relationships can be restored following perceived bias.

Comparison of functional benefits of self-management training for amputees under virtual world and e-learning conditions.
This project used a randomized design to compare two methods of disseminating an evidence-based self-management intervention for amputees, which included, avatar-based virtual world and e-learning environments. The investigators found that the virtual world group had a significantly higher dropout rate than the e-learning group.

Rethinking the outpatient medication list: increasing patient activation and education while architecting for centralization and improved medication reconciliation.

Feasibility of implementing group well baby/well woman dyad care at federally qualified health centers.
The aim of this qualitative study was to assess stakeholder perspectives on the feasibility of implementing Centering Parenting (CP) in federally qualified health centers (FQHCs) in Baltimore. The investigators concluded that perceptions regarding facilitators and barriers to CP implementation in FQHCs were similar to existing group well-child care literature; however the benefit of emphasis on maternal wellness was a unique finding. The investigators suggested that a maternal wellness integration might make CP a particularly desirable model for implementation at FQHCs, but potential systems barriers must be addressed.

A decision aid to promote appropriate colorectal cancer screening among older adults: a randomized controlled trial.
The purpose of this study was to test a patient decision aid (PtDA) to promote CRC screening in older adults. The PtDA was designed to facilitate individual decision-making to help patients understand the potential risks and benefit of CRC screening. 424 patients aged 70 – 84 who were not up to date with CRC screening participated in a double-blinded randomized controlled trial, using the PtDA or an attention control. Two outcomes - appropriate CRC screening behavior at 6 months following the initial visit or appropriate screening intent immediately after the visit - were defined as completed screening or intent for patients in good health, discussion about screening with their provider for patients in intermediate health, or no screening or intent for patients in poor health. Appropriate screening behavior at 6 months was higher in the intervention group; appropriate screening intent following the provider visit was also higher. The authors conclude that the PtDA for older adults promoted appropriate CRC screening behavior.

Patient portal usage in pediatric urology: is it meaningful use for everyone?
This study examined pediatric urology patient portal enrollment and activation patterns at a tertiary pediatric hospital in Southern California by race/ethnicity, preferred language, gender, and residential region. The study concluded that primary language and socioeconomic factors may be significant barriers to portal adoption. Patient education to reduce these barriers may increase portal acceptance and increase meaningfulness to the portal for patients/parents and providers.

"I don't want to take chances.": a qualitative exploration of surgical decision making in young breast cancer survivors.
The purpose of this study was to better understand the choice of contralateral prophylactic mastectomy (CPM) through a qualitative exploration of surgical decision-making in young breast cancer survivors, including how issues particular to younger women affected their decision and the post-surgical experience. Through focus groups, themes emerged and were categorized. The authors concluded that informational resources and decision aids may enhance patient-doctor communication and help young survivors better understand risk and manage expectations surrounding short- and longer-term physical and emotional effects after surgery.

One year follow-up of outcomes from the randomized clinical trial of the building on family strengths program.
This study tested the 12-month efficacy of an inclusive non-diagnosis-specific, parent education program with seven in-person sessions. The outcome measures were self-efficacy, parent and child shared management of chronic condition, coping skills, parental depressive symptoms and quality of life. All of the outcomes improved within the intervention group over 12 months.

Parent perspectives on information about late effects of childhood cancer treatment and their role in initial treatment decision making.
The authors’ objective was to explore how parents of children with cancer consider late effects in initial treatment decision making and during active cancer treatment. Through interviews, they found that parents desire detailed information about late effects to make informed treatment decisions and to prepare for the future. Despite the role of late effects in treatment decision making, some parents feel that late effects are either inadequately addressed or too overwhelming to process at diagnosis. They suggested that parents may benefit from early assessment of their information needs and a return to these issues over time.

Social media vaccine websites: a comparative analysis of public and moderated websites.
This study examined vaccine social media (VSM) websites and compared expert moderated VSM websites to nonmoderated websites. Accuracy of information, vaccine stance, and tone was coded for 79 public blogs and discussion boards and 61 expert moderated VSM websites. The public VSM websites had a lot of inaccurate information and tended to have a negative stance towards vaccines. Expert moderated websites had a more civil tone, little to no inaccurate information and very little participant-to-participant interaction.

Evaluation of a multimodal, direct-to-patient educational intervention targeting barriers to osteoporosis care: a randomized clinical trial.
Osteoporosis treatment rates are declining, even among those with past fractures. Novel, low-cost approaches are needed to improve osteoporosis care. In this study, the investigators conducted a parallel group, controlled, randomized clinical trial evaluating a behavioral intervention for improving osteoporosis medication use. They found that the primary study outcome was self-report of osteoporosis medication use at 6 months. Other outcomes included calcium and vitamin D supplementation, bone mineral density (BMD) testing, readiness for behavioral change, and barriers to treatment.

Standards for UNiversal reporting of patient Decision Aid Evaluation studies: the development of SUNDAE Checklist.
This study sought to develop and reach consensus on reporting guidelines to improve the quality of publications evaluating patient decision aids (PDAs). The study concluded that the SUNDAE (Standards for UNiversal reporting of patient Decision Aid Evaluations) Checklist will help ensure that reports of PDA evaluation studies are understandable, transparent and of high quality.

Expanding the Facebook platform to engage and educate online communities.
This article discusses the development and use of a mobile application (app) called Liver Space that was developed through Facebook’s platform. This app is for the pediatric liver community including patients and caregivers. Unlike most Facebook health groups, this one is vetted by healthcare providers who are specialists. The app provides up-to-date information and includes emerging news, summaries from important scholarly journals and human interest stories. There is an “ask an expert” function incorporated into Liver Space. Also included in the app is the ability for users to track their labs and weight and to graph the data.

A community engagement method to design patient engagement materials for cardiovascular health.
This trial included development of locally tailored cardiovascular disease (CVD) patient engagement materials through Boot Camp Translation (BCT), a community engagement process that occurred before practice recruitment but after cluster randomization. The 4 BCTs' messages and materials developed by the BCT groups uniquely reflected each community and ranged from family or spiritual values to early prevention or adding relevance to CVD risk.

Development of new quality measures for hospital-based care of suicidal youth.
This study researched the value of 4 new quality measures developed to assess hospital-based care for suicidal youth. The four quality measures focused on counseling caregivers about restricting access to lethal means of self-harm, and the benefits and risks of antidepressant medications. They were divided into measures for the emergency department (ED) and inpatient measures. Survey field tests were conducted with caregivers of youth who were admitted to the ED or inpatient care for suicidality at 1 of 2 children’s hospitals between July 2013 and June 2014. Most caregivers did receive counseling about restricting their child’s access to lethal means of self-harm and also reported higher rates of counseling of benefits on antidepressants both in the ED and in the inpatient setting than the risks.

Consulting Dr. Google: quality of online resources about tympanostomy tube placement.
The authors of this study evaluate the quality of leading Internet resources describing tympanostomy tube (TT) placement. They concluded that Internet resources about TT placement varied in quality pertaining to health literacy, principles of shared decision making, and consistency with practice guidelines.

Development and pilot testing of multimedia patient education tools for patients with knee osteoarthritis, osteoporosis, and rheumatoid arthritis.
Researched developed multimedia educational tools that incorporated videos to help patients understand their chronic diseases including osteoporosis, osteoarthritis, and rheumatoid arthritis. Tools were developed in English and Spanish. A total of 60 patients (20 per disease) were recruited to see the videos and were interviewed after all. All participants gave them a favorable rating and felt it helped them understand the disease, the importance of taking their medications, and seeing their doctor regularly. They found the videos and accompanying materials easy to use and easy to understand.
