This section includes tools and resources that AHRQ developed, in collaboration with experts in primary care, to advance primary care research and improve the quality and safety of primary care. These tools and resources help researchers and evaluators to design and evaluate primary care research, decision makers to implement practices for primary care transformation, and clinicians to enhance the delivery of primary care by improving care teams, care processes, and achieving best practices.

1 to 3 of 3 Tools and Resources DisplayedThis update of the 2011 Care Coordination Measures Atlas presents many new measures emphasizing primary care. Twenty-six new EHR-based measures are identified that can help professionals meet Medicaid and Medicare EHR Incentive Programs criteria.

The Clinical-Community Relationships Measures (CCRM) Atlas was developed to identify ways to further define, measure, and evaluate programs based on clinical-community relationships for the delivery of clinical preventive services. The CCRM Atlas uses a framework to examine the functioning and effectiveness of clinical-community relationships.
