Effect of health system on the association of rurality and level of disadvantage with receipt of diabetic eye screening.
The goal of this retrospective cohort study was to determine the role of level of disadvantage in diabetic eye screening to explain the effect of health systems on rural and urban disparities. Researchers used an all-payer, statewide claims database to include adult Wisconsin residents with diabetes who had claims billed throughout the baseline and measurement years. Results indicated that patients from urban underserved clinics were more likely to receive screening than those from rural underserved clinics; similar findings emerged for both Medicare and non-Medicare subgroups. The researchers concluded that health systems, especially those that serve urban underserved populations, have an opportunity to increase screening rates by leveraging health system-level interventions and supporting patients in overcoming barriers.

Effects of program scale-up on time to resolution for patients with abnormal screening mammography results.
Effects of geographic program expansion to rural areas on mammogram screening program outcomes are understudied. The authors of this study sought to determine whether time-to-resolution (TTR) varied significantly by service delivery time period, location, and participant characteristics across 19 North Texas counties.

Disparities in the use of diabetes screening in Appalachia.
This study examines disparities in the use of diabetes screening in Appalachia. Results showed that at-risk counties had significantly lower screening rates than competitive counties. Recommendations include introducing social policies that improve socioeconomic status and educational attainment, and health policies that reduce barriers to access to care in order to reduce disparities in diabetes screening rates in the less affluent Appalachian counties.

A systematic review of clinic and community intervention to increase fecal testing for colorectal cancer in rural and low-income populations in the United States - how, what and when?
Researchers conducted this systematic review to determine how implementation strategies and contextual factors influenced the uptake of interventions to increase fecal testing for colorectal cancer in rural and low-income populations. They found that provision of kits through the mail, use of pre-addressed stamped envelopes, client reminders and in-clinic distribution appeared most frequently in the highly effective/effective clinic-based study arms. Few studies described contextual factors or implementation strategies.

"Finding the right FIT": rural patient preferences for fecal immunochemical test (FIT) characteristics.
This community-led study was conducted to assess patient preferences for fecal immunochemical tests (FIT) characteristics and to use study findings in concert with clinical effectiveness data to inform regional FIT selection. The authors concluded that FIT characteristics influenced patient's perceptions of test acceptability and feasibility and indicated that health system leaders, payers, and clinicians should select FITs that are both clinically effective and incorporate patient preferred test characteristics.

Assessing local capacity to expand rural breast cancer screening and patient navigation: an iterative mixed-method tool.
To expand rural breast cancer screening , the researchers proposed a regional hub-and-spoke model, partnering with local providers to expand access across North Texas. Their capacity assessment tool enabled the hub to establish partnerships with county organizations by tailoring support to local capacity and needs.

Disparities in receipt of breast and cervical cancer screening for rural women age 18 to 64 with disabilities.
The authors examined the combination of disability status and rurality in association with receipt of breast and cervical cancer screening among women age 18 to 64 in the United States. They found that women with disabilities were less likely to be up to date with mammograms and Pap tests compared with women with no disabilities, and women in rural areas were less likely to have received breast or cervical cancer screening within recommended timeframes. Women with a disability who lived in a rural area were the least likely to be current with screening.

Using a discrete choice experiment to inform the design of programs to promote colon cancer screening for vulnerable populations in North Carolina.
This study used a discrete choice experiment (DCE) to learn about how vulnerable individuals in North Carolina value different aspects of CRC screening programs. It found that follow-up cost coverage was most frequently found to be the most important attribute from the DCE (47 percent); followed by test reward/copayment (33 percent).
